Remote Patient Monitoring (RPM) and Chronic Care Management (PCM/CCM) allows practices to better track patient care, monitor therapeutic benefit, and identify problems before they become serious. In this time of rising opioid use and opioid-related mortality, the Pain Scored solution is both timely and essential.
The Pain Scored Service is a reimbursable platform that provides standardized measurements of pain, activity, and mental health over specific time points in between doctor visits. The data is collected, interpreted and evaluated. Reports are used by healthcare providers to make better treatment decisions using evidence-based assessments that provide a holistic picture of health. Pain Scored can be thought of as a two-way (patient and physician facing) “pain diary” that helps patients take charge of their health comprehensively in collaboration with their providers. Providers will be able to assess their patients based on data between office visits as opposed to a snapshot of time at the visit. This allows for faster intakes and medication reconciliation. For patients, we help them explain their story quickly in “a way that providers can understand”.
