Maryland Honoring Choices Coalition

 


We know that naming a health care agent and/or completing an advance care plan helps health care providers honor wishes when patients are unable to speak for themselves.  And we know that communities most successful in honoring patient wishes have created systems that regularly present residents with the opportunity to complete an advance care plan.

OUR APPROACH is to build a coalition of members who support this goal and who endorse our efforts to implement the recommendations of the State Advisory Council on Quality Care at the End of Life to increase the completion of advance care plans in Maryland.  Our coalition will build relationships and connect with key stakeholders over 2021 so we can seek successful legislative changes during the 2022 session.

JOIN US AND HELP REALIZE OUR VISION WHERE ALL MARYLANDERS ARE EMPOWERED – AND ENCOURGED – TO MAKE THEIR CARE WISHES KNOWN, AND WHERE THOSE WISHES ARE HONORED.

By joining the coalition…

I/my organization agree that regularly presenting residents with opportunities to name an agent and/or complete an advance care plan is the best way to ensure care wishes are known and honored.

I/my organization support the coalition's action to implement the recommendations of the State Advisory Council on Quality Care at the End of Life. This includes support for options like:

  • Requiring, encouraging, or incentivizing insurance companies to offer advance care planning as a covered benefit; and/or presenting opportunities to complete an advance care plan during open enrollment;
  • Embedding advance care planning into health system workflows, with a particular sensitivity to underserved communities;
  • Making it easier to complete, upload, and retrieve electronic advance directives;
  • Presenting the option to complete an advance care plan when residents access state services (e.g., when registering a car or renewing a driver's license); and
  • Encouraging state leaders to publicly recognize National Healthcare Decisions Day, and supporting existing public awareness campaigns.
