Weekly Update 5/20/2019

  • The MDPCP registration period has opened for 2020.
    • You do NOT need to reapply.
    • You will be required to sign a new Participation Agreement sometime in the Fall for 2020.
    • If you know of any practices who were deferred last year or did not apply, please encourage them to apply for 2020.
  • You should have all received your 2nd Quarter Care Management payments from CMMI in late April. IF you haven’t received this payment, please reach out to us.
  • Amalia has sent you all SBIRT evaluations to complete and return to her.
    • Once completed, someone from Mosaic will be contacting you to set up an initial call and to schedule a time for in office training.
  • Please see information regarding Annual Wellness Visits including what screenings can be completed within the AWV and how often.
  • There is a MDPCP webinar this Thursday, May 23rd at 5pm.
    • The topic is: Quality Improvement and eCQM.

  • Maryland law requires all new and renewal applicants for the Maryland CDS Registration issued by Office of Controlled Substances Administration (OCSA) to attest to completing 2 Hours of Continuing Education, related to the Prescribing or Dispensing of Controlled Dangerous Substances. MedChi has created a 2 Credit Prescription Drug Monitoring Program CME and is presenting it around the state:
    • Washington County - May 23
    • Carroll County - June 5
    • Western Maryland - June 19
    • Anne Arundel County - June 20
    • Charles County July 24
    • Mid-Eastern Shore - August 21
    • Calvert County - September 11

The MDPCP Program Management Office (PMO), in collaboration with Medicalincs LLC, is pleased to bring you an in-person, FREE MDPCP Advanced Primary Care Staff Training. This comprehensive, hands-on, interactive, one-day training program is designed to enhance your practice staff’s expertise with the functions of Advanced Primary Care. The training will bolster your ability to support your practices in implementing change to meet the MDPCP care transformation requirements, and ultimately patient health and experience outcomes.

In addition to networking with other MDPCP practice staff, the training session topics will focus on key areas of care coordination & population health management (including applications & tools). The training will be presented by the subject matter experts and the PMO team – in a practical workflow & journey format. Key topics will include identification, revision, and management of - Empanelment, Risk Stratification, Transitional Care Management, Behavioral Health Integration, Registry Management & Care Alerts, Social needs, and associated Data Reviews & Administrative Support.For your convenience, we are offering the same training at 5 different locations in different regions across the state on Fridays and Saturdays. All training materials you need will be provided to you at NO COST. Please register early, as space will fill up quickly.     Weekly Update 4/9/2019

  • We are rolling out the following services to our CTO practices:

    • Backline®
      • A secure messaging solution built specifically for healthcare that meets all HIPAA, HiTech Act and Joint Commission requirements, in order to text message with colleagues and patients, without the risk of sending protected health information (PHI) over common, non-secure channels.
      • Real-time delivery status indicators and timestamps – including CRISP ENS Alerts.
      • The CTO is paying for Backline access for the identified Care Manager for each practice
      • Your Care Manager will be contacted by the Backline team from DrFirst for Backline registration, training and workflow assistance.
    • Screening, Brief Intervention, and Referral to Treatment (SBIRT)
      • Some of you have been contacted to complete an SBIRT readiness assessment. We are rolling it out based initially on EHR. The vendor hired by the MDPCP, Mosaic will assist you in both practice and EHR workflow. We anticipate all practices will be trained in SBIRT in 2nd Quarter.
    • Diabetes – we will be starting a diabetes management pilot program shortly – more info to come soon. In the meantime, please visit the MedChi CTO website for diabetes management information.

    There are several upcoming webinars you may wish to join – please note the CRISP webinar on May 7th that is specifically for our CTO practices:

    • MDPCP Care Management Webinar - During this session, you will have an opportunity to hear and learn care management implementation strategies from other MDPCP practices and identify best practices. You are encouraged to  participate in this event regarding your practice’s experiences and plans for implementing and integrating care management.

    • The questions you will be required to answer are on the attached document (First Quarter 2019 Reporting Questions), so you may review them prior to starting your reporting.

    MedChi as a Care Transformation Organization for the MDPCP:

    MedChi is partnering with the Maryland Department of Health to promote and assist with the new Maryland Primary Care Program (MDPCP), which launched January 1,  2019. MedChi has been approved as a physician led Care Transformation Organization (CTO). The role of a CTO is to provide support to practices via care management personnel, infrastructure, and technical assistance. Providers are not required to contract with and receive services from CTOs, though the State expects many providers would benefit from the education and technical assistance tailored to their needs.
