Weekly Update 3/18/2019

• If you continue to have any problems on accessing any of the portals, please reach out to us so we may assist. These portals include:

  • CMS MDPCP Portal
  • CRISP Reporting Portal: you should have received an email invitation from CRISP for this portal. If you have not, please contact us
  • MDPCP Connect

• If you have not already done so, you must complete the attached Care Coordination Affiliation Statement – this gives CRISP permission to share your practice data with the CTO

• You must notify your Medicare attributed patients of your participation in the MDPCP. Attached is a template letter for that communication.

  • Please copy and paste the body of the attached onto Practice letterhead
  • You should post this in your waiting room, website and on your patient portal
  • You are required to mail or electronically send to all your attributed beneficiaries

• We have met with Mosaic, the group the state has contracted with to conduct SBIRT trainings. We will be contacting each practice individually to complete a Readiness Assessment and get the trainings scheduled.

MedChi as a Care Transformation Organization for the MDPCP:

MedChi is partnering with the Maryland Department of Health to promote and assist with the new Maryland Primary Care Program (MDPCP), which launched January 1,  2019. MedChi has been approved as a physician led Care Transformation Organization (CTO). The role of a CTO is to provide support to practices via care management personnel, infrastructure, and technical assistance. Providers are not required to contract with and receive services from CTOs, though the State expects many providers would benefit from the education and technical assistance tailored to their needs.
