The MDPCP COVID-19 Vaccine Pilot is in full swing – if you are interested in participating and you are not yet registered to become a vaccinator, please review the email sent by Grace (gpark@medchi.org) last Wednesday 3/31 for the next steps.  More details are in the COVID-19 Updates section below.

  3. “State of the MDPCP” All-Practice Call 
    Join the MDPCP Program Management Office on April 23rd for the first All-Practice Call where they will discuss the performance of the MDPCP program in key metrics and high-level strategy for the coming year.  They will also solicit your feedback.  All practices should join to understand the present and future state of the MDPCP, and how your practice fits in.

  4. Senior Tablet Program    
    This is another reminder that your patients can receive a free, internet-enabled tablet.  We highly recommend utilizing this service for any patients who are having difficulties with telehealth or connecting virtually to loved ones and caregivers.

COVID-19 Updates                 


  1. Vaccinations for MDPCP Practices   
    March 18th, Governor Hogan announced the beginning of vaccinations in MDPCP practices.  MDPCP has started a pilot with 37 of your MDPCP colleagues’ practices.  The Governor has committed to expanding this pilot very quickly. MDPCP hopes that all practices will be willing and able to join in this effort by meeting the following requirements:   

    - Ensure you are fully onboarded to ImmuNet by following the steps in this flowchart based on your current status-
              manual reporting is not 
    allowed   
    - Complete registration form to request to be a COVID vaccinator   
    - Have proper refrigeration - dorm style refrigerators are not permitted
       

    Full COVID vaccinator registration will allow you to be contacted by MDPCP in the weeks to come to establish your practice as a COVID vaccine site.  In the meantime, please solidify your plans for vaccinating your patients; clearly define outreach, scheduling, vaccine storage, administration and personnel.

  2. Ask Dr. Haft Series - Starting tomorrow, April 7th 
    Ask Dr. Haft all your questions in the PMO’s new Ask Dr. Haft Series!  The first session will open tomorrow, April 7th, and focus on the vaccination effort.  Look out for a Connect post indicating the session has opened and submit questions to the Q&A Forum within 48 hours.

  1. eCQM Reporting – Action Required by tomorrow!  

    Quality Measure Reporting in CRISP ULP closes tomorrow, March 31stFailing to submit your eCQM data before the deadline will result in your practice forfeiting your PBIP payment and can prevent Track 1 practices from transitioning to Track 2.

Tips for Completing Financial Reporting  

  • Review questions. Collect data. Complete report.   
    • We recommend that you go into the portal, review all asked questions without entering any data. Then use your practice financial statements to answer the questions in the portal.  
  • Remember MDPCP Expenditures cannot exceed the MDPCP Revenue.   
    • Double check your Expenditure and Revenue numbers on the summary slide to ensure the Total MDPCP Expenditures (circled below in blue) are less than Total MDPCP Revenue (circled below in red).

    The Maryland Health Care Commission (MHCC) seeks your response to a brief questionnaire about telehealth.  Your response will help 

     inform future telehealth policy.  The questionnaire takes only five minutes to complete.  Your response to all of the questions is

  3. Senior Tablet Program *New 
    The Maryland Department of Aging and EagleForce Associates are partnering to meet the needs of older adults without access to digital services necessary to participate in telehealth and online social and health promotion programs.  There will be a webinar on Friday March 12th at 12PM to learn about the available tablets and support services, how you can refer a client in need, and how to prescribe and bill for the service.  This may be a great option for those patients that have issues with access/telemedicine.  You may register HERE or in the Additional Webinars section below.  



  4. Practice Reporting    
    Practice Reporting opens March 22nd and closes on April 9th.  Due to the short window to complete this reporting, we ask that all of our practices take a look at the Practice Reporting Sample Questions prior to the reporting period.

    Please reach out if you have any questions about how to answer these sample questions, and we would be happy to go over them with you.

  5. CAHPS Survey   
    MDPCP has begun mailing surveys to randomly-selected patients from your CAHPS Patient Roster. Please encourage your patients to complete the CAHPS Survey. Not only does the survey provide you with valuable information, but it also makes up a portion of your PBIP score.

  1. Steps You MUST Take to Receive COVID-19 Vaccine for Office Distribution

  2. New Vaccination Sites *New 
    Governor Hogan announced three new vaccine sites opening this month, and an increase in distribution for vaccines at the M&T Bank Stadium.  HERE is the link to view COVID-19 vaccination locations and register.
  • Southern Maryland: Will open at Regency Furniture Stadium on March 4.  Initial appointments were made available on March 2.
  • Eastern Shore: Will open at Wicomico Youth & Civic Center in Salisbury no later than March 18. Tidal Health will serve as the site's clinical partner.
  • Western Maryland: Will open by the end of the month.  The site will be located at the Hagerstown Premium Outlet, and Meritus Hospital will be its clinical partner.
  • Baltimore City: The governor announced that the M&T Bank Stadium mass vaccination site will ramp up ahead of schedule this week to 2,000 shots per day. The Baltimore Convention Center site will focus on underserved populations and ramp up to more than 1,000 shots per day.

      The MedChi CTO has partnered with four companies that provide remote health management technology to MDPCP Patients.  Please see the attached RPM Offerings document to learn more.  You are able to utilize multiple RPM companies.

    2. SBIRT Stipend
      We have an opportunity for our practices to utilize Mosaic Group's services to implement Screening, Brief Intervention & Referral to Treatment (SBIRT).  It is completely free and very easy to work with Mosaic Group, and SBIRT meets MDPCP's Behavioral Health Integration Requirement.  You can receive a $1000 stipend to incorporate SBIRT into your practice, but this opportunity will only available through the end of June.

    3. Practice Reporting *New
      Practice Reporting opens March 22nd and closes on April 9th.  Due to the short window to complete this reporting, we ask that all of our practices take a look at the Practice Reporting Sample Questions prior to the reporting period.

      Please reach out if you have any questions about how to answer these sample questions, and we would be happy to go over them with you. 

    4. CAHPS Survey
      MDPCP has begun mailing surveys to randomly-selected patients from your CAHPS Patient Roster.  Please encourage your patients to complete the CAHPS Survey.  Not only does the survey provide you with valuable information, but it also makes up a portion of your PBIP score.

    1. Steps You MUST Take to Receive COVID-19 Vaccine for Office Distribution

      1.  Fill out the COVID Vax Clinic Info Form 
      AND
      2.  Complete the ImmuNet COVID-19 Vaccinator Registration as soon as possible

      The Maryland State government will use the information practices have submitted along with the ImmuNet registration list in order to determine which practices receive COVID-19 vaccines.  Read the clinician letter here.  Registration completion does not mean the vaccine will be immediately available for ordering.  Once ordering is open for all providers in ImmuNet, notification will be sent to all registered providers

      Based on the success of the first round of pairings, MDPCP is launching another round of pairings for the Buddy Program.  If you are interested in joining the program for our next round of pairings, fill out this short survey.  The survey will allow MDPCP to match practices based on same starter year, practice size, and similar preference for care transformation focuses.

      Quarter 1 Payment and Attribution was updated on 1/29 in the CMS Enterprise Portal (MDPCP Portal).  Please verify the payment amount in the portal matches what you have received in your bank account.   

      As a reminder, please download your new attributed beneficiary list and review any changes to your MDPCP patient population.

      CMS has made some slight updates to the PY2021 Payment Methodologies document you can find attached to this email or download here.

      These changes include:  
      - The addition of some codes that count towards attribution  
      - An update to the way HCC scores are determined (footnote on page 21)

     2. Monoclonal Antibody Referral *New

         Referrals to infusion sites for monoclonal antibodies are now available to be sent through CRISP!  As of Thursday 1/21, all
         providers with CRISP ULP access now have the "Referrals" tab to make referrals to infusion sites.

         This workflow is live at 6 infusion sites currently and you can start sending referrals today (Baltimore Convention Center is not using the
          workflow, use this link instead; recently added infusion sites will be onboarded to CRISP).  Infusion sites will receive these referrals
          through CRISP and follow through with the patient.

      The CAHPS Survey deadline has now been extended to this Friday, January 22nd. Please find the status of your roster submission in the table below.


      Not Submitted: Sign up for a meeting with the MedChi CTO to pull a patient roster, we have added spots through Friday 1/22.  Please sign up using the link HERE.
      Submitted: Monitor your email for further communication from CMS.
      Complete: No further action is needed. 



      The new correction period is January 25th – January 29th.  This correction period is only for rosters that were already submitted by the 22nd but had errors.  You will get an email from CMS once your roster is considered complete or if edits are needed.

    • MDPCP Buddy Program *NEW, deadline 1/25
      We would like to highlight this year's Buddy Program: a volunteer based program where experienced practices and new practices are paired up together to share insights and experiences with one another.     • eCQM Reporting
      Quality Measure Reporting is open in CRISP ULP through March 31st.  Please run a 2020 (12-month) report and capture the numerator, denominator and possible exclusions for both CMS 122 and CMS 165.  You can submit this data in the “2020 eCQM” tab (for manual entry) or the CAliPR tab (for QRDA III files) in your Unified Landing Page.  We encourage you to report at a practice level, but you can report under individual NPIs if that is preferred.  If you have questions or concerns, please email Daphne at dbrannon@medchi.org.

      Registration through ImmuNet to receive the COVID-19 vaccine is now open for primary care providers as of Thursday, January 7, 2021.

      You must:     
      1.  Enroll in ImmuNet
      2.  Set up EHR vaccination data reporting to ImmuNet   
          
      Primary care providers will need to complete the CDC COVID-19 provider agreement through their existing VFC profile if applicable or in a new profile if non-VFC (paper copies of the agreement will not be accepted). Please review the Quick Reference Guide: COVID-19 Vaccine Registration & Ordering (beginning page 2 for Non-VFC; page 5 for VFC) before attempting to complete your registration.

      COVID-19 vaccine is still in very limited supply and the State of Maryland is vaccinating according to ACIP phase 1A recommendations (front line healthcare workers and residents/staff of LTCFs).  If you are seeking COVID vaccine for yourself or staff please contact your local health department.  Once COVID-19 vaccine is available for ordering in ImmuNet you will be notified.

    • CAHPS Survey Roster   
      The CAHPS Survey deadline is this Friday, January 15th.  All practices are required to generate and submit a Patient Survey Roster to CMS during the submission period of January 4, 2021 – January 15, 2021.  

      If you have not signed up for a meeting with the MedChi CTO to pull a patient roster, there are a few spots left on Thursday 1/14 and Friday 1/15.  Please sign up using the link HERE.  

       Please notify the MedChi CTO by emailing cto@medchi.org once you submit your patient roster, as we cannot track the submissions.  

      Important Notes:   
    • The Patient Survey Roster should include information for all eligible adult patients who had a visit in person, by phone or by video (scheduled or walk-in) by an eligible provider from July 1, 2020 to December 31, 2020.
    • Files may be submitted as an Excel file or as an Excel CSV file. Either file type must follow the sample specifications described in the Patient Survey Roster file layout.
    • The Patient Survey Roster files will be checked for accuracy and completeness. If a file is found to be inaccurate or incomplete, the Lewin Group will reach out directly to the practice to correct and resubmit the Patient Survey Roster file until the end of the correction period (January 18th - January 22nd).
    • Data fields should not contain words such as “N/A,” “Missing,” or “NULL.” If data is not available, please leave the field blank. Dates should be in the format MMDDYYYY with no separators, and phone numbers should be 10 digits with no parentheses, dashes, or any other delimiter.
    • *Please see the attached roster template and instructions for further details. There are specific instructions for both generating and submitting the necessary roster in the appropriate format.

      If your practice receives notice that you were selected to be audited for 2019, please email cto@medchi.org to let us know.  We, as a CTO, are not notified of audit selections, so we will need to be notified by any selected practices in order to assist you.  

      Please note that this only applies to practices and CTOs that participated in PY 2019, however, PY 2020 participants may want to review the attached documents for an understanding of what to expect for PY 2020 audits.

    • *NEW 2021 Coding Updates and ChangesThursday, December 10th (12-1:15PM)
      There are big changes coming up for Evaluation and Management (E&M) coding, starting in January 2021. History and Physical Exam Components will no longer be 2 out of 3 factors to determine the Level of CPT code. Medical Decision Making or Time will play the key role to determine the Level of CPT code

      The webinar will include a 30-minute presentation, covering major 2021 E&M changes, documentation requirements for history and physical exam components, in-depth review of Medical Decision Making and Time, added, deleted, and revised codes. Several encounter notes will be presented as examples for the new way of coding for E&M codes. After the presentation, there will be a Live Q&A. The link can be found here

    • Connect Single Sign-On
      MDPCP Connect was upgraded to Single Sign-On (SSO) on Monday, November 2nd. SSO allows users to login with one set of user credentials across all CMS Innovation Center applications, and enhances the security of your confidential information along with CMS beneficiary information.  If you haven’t already, upgrade your account for single sign-on so you can get back on Connect!

      Steps to Complete SSO

    • Be sure to use the new URL: https://app.innovation.cms.gov/CMMIConnect
    • If you were a MDPCP Connect user prior to 11/2, click “Existing User Verification”
    • If you were not a MDPCP Connect user prior to 11/2, click “New User Registration”
    • As a tip, your IDM account credentials will be what you use to login into the CMS Portal

      The MDPCP PMO is collaborating with ImmuNet to verify MDPCP practices’ reporting status.  If your practice is not enrolled in ImmuNet, please enroll using the links below.  Your MDPCP coach will reach out with information on your practice’s status with ImmuNet.

      All practices should: 
    • After Day 7 with a Negative Test
    • If no symptoms are reported during daily monitoring
    • AND if a diagnostic specimen tests negative
    • After Day 10 Without a Test

    • *New Up to $60,000 Relief Assistance for Practices in Montgomery County

      The Montgomery County Primary Care and Dental Grant Program will be providing grants for independent primary care medical and general dentistry practices that have lost revenue and have incurred re-opening costs related to the COVID health crisis.

      The application and instructions will be posted here tomorrow 11/18. More information can be found here.

      Eligibility: 
    • Be a primary care medical or general dentistry practice that is not financially supported or owned by a commercial organization or large anchor organization. Practice clinicians must own a majority of the practice and retain key decision-making authority. 
    • Have at least one location in Montgomery County primarily used for treating patients. 
    • Have experienced a decrease in operating income attributable to the COVID-19 public health crisis that has not been previously reimbursed via another local, State or Federal program. 
    • Be in good standing with the State of Maryland. All providers must also be in good standing with the appropriate state licensing agency.

      If you would like to change your CMF Split for 2021, you must send an email stating your updated split to MarylandModel@cms.hhs.gov following proper formatting.

      Payment and attribution data for Quarter 4 2020 was updated in the MDPCP Portal on October 30, 2020.  Please check the portal to view your Q4 data.

      Under the CARES Act, sequestration is not being applied for any payment or partial payment occurring for the months May through December 2020.  Please see the PY2020 Payment Methodologies document for more information about how sequestration is applied in MDPCP.
